Q: Why do you believe it is important to proactively diagnose periodontal disease?
A: There are so many systemic diseases linked to periodontal disease and inflammation as a result of perio disease. I feel it is our duty as health care providers to be proactive and remember that there is a whole body attached to the oral cavity we are treating. If I can help even one patient lower their blood pressure or control their diabetes, it is a good day.
Q: What do you believe is the appropriate protocol for diagnosing periodontal disease?
A: We need to utilize our tools such as full series of radiographs to determine bone height, full mouth periodontal charting, recession, and clinical exam. In my office, full mouth probings are completed/charted at every recare appointment. If we detect changes, treatment is recommended immediately.
Q: What are some of your protocols for treating perio disease?
A: We always begin treatment with scaling and root planing. Our patients are seen 6 weeks after their final SRP for evaluation, reprobing, scaling, and polishing. They are given very thorough home care instructions and placed on either a 3-month recare or 4-month recare depending on results, bleeding, etc. If there is no improvement in probings, home care, or bleeding, we then refer to our periodontist for possible surgical intervention.
Q: What treatment hurdles do you encounter and how do you overcome those hurdles?
A: The biggest hurdle is home care compliance and resistance to periodontist referral. Our trick for both is consistency. We reiterate at every recare appointment the need for further treatment and changing their home care habits. I have found that, eventually, consistency pays off and the patient will finally visit our favorite periodontist.
